Registration Forms

Registration forms for the 2010 soccer season are now available for download and printing from this site or may be obtained by contacting the club president at 604-803-0705.

Complete the registration form and submit it with the required cheque and documentation. The registration fee is $150.

Please ensure that all forms are complete and all required documentation is included. Incomplete packages will result in delays during the registration process.

Required Supporting Documentation

All players entering U7 this season through U18 must submit proof of birth date. This should be a photocopy of the player's birth certificate, passport, and / or citizenship papers.

Please ensure that the proof of birth documentation is included with the submitted registration package.

Registration Checklist

Registration form completely filled out and signed by parent or guardian.
Registration fee cheque attached.
Proof of birth (as indicated above) attached.
